This letter serves as formal notification of my/our intent to homeschool my/our child(ren) as stipulated under the Utah Code Ann. §53G-6-204. I/we understand the legal requirements to provide an instruction equivalent to that offered by the state's public schools and intend to adhere to these standards. The following child(ren) will be homeschooled during the upcoming school year:
